Please submit your chapter convention registration form electronically using the form below no later than two weeks prior to the convention (March 10, 2022)

 Please submit only one registration per chapter and complete only the fields marked by a red asterisk.

Please list all members attending (First Last, First Last....) as well as any special physical or dietary needs:

Registration Fee:

 - Registration fee is $12 per person. Make Checks Payable to McKendree University and place Sigma Zeta on the memo line of the check. Fees can be paid in advance by mailing or can be paid during registration. You can also pay with credit card (see credit card authorization form)

Mail checks payable to: McKendree University. If you are in need of a W-9 form to pay the registration fee for the National Convention, please contact Sigma Zeta National (sigmazetanational@mckendree.edu).
